Important Information and Instructions
Thank you for scheduling your EMG / NCS appointment with Dr. Schulman.
aquí para españolPlease complete the New Patient Registration Form at your earliest convenience.
IMPORTANT INFORMATION ABOUT YOUR APPOINTMENT, PLEASE READ EVERYTHING BELOW. IF THE INSTRUCTIONS ARE NOT FOLLOWED, YOUR APPOINTMENT MAY NEED TO BE CANCELLED.
- If you have any open wounds, sores, rashes, skin infections, or recent surgical incisions anywhere on your body, please notify the office as soon as possible prior to your scheduled appointment. Your test will need to be rescheduled until your skin is fully healed to avoid any possibility of infection.
- Please shower or bathe the day of or evening before your appointment.
- If you are having a lower extremity study (legs/feet), make sure you wear shoes and long socks that cover your calves (no sandals without socks – feet and legs need to be warm before the test) and wear loose-fitting long pants that can roll up to the knee. If you have any swelling on your legs or feet, please contact our office, as swelling can prevent the test from being performed accurately.
- If you are having an upper extremity study (arms/hands), please make sure you wear a short sleeve shirt. If you have any swelling on your arms or hands, please contact our office, as swelling can prevent the test from being performed accurately.
- DO NOT apply skin lotion or any oils to the skin as this will interfere with the ability to perform the test properly. This includes oily body washes and oily soaps. Your appointment may need to be canceled if lotion or oils are applied.
- DO NOT wear any cologne/perfume or heavily scented products as this affects other patients with sensitivities or migraines. Deodorant is okay.
- If you take any medication, please continue to take all of them as you normally would.
- If you use a hearing aid, be sure to bring it/wear it to your appointment, so that you can communicate effectively with the doctor.
- If you have a spinal cord stimulator, or a deep brain stimulator, please be certain to bring the controller so that the device can be turned off during the testing procedure. The electrodiagnostic test cannot be performed if the stimulator remains on, due to interference from the stimulator.
- Please arrive promptly at your arrival time so that we can check you in. If you have not completed the New Patient Registration Forms online, then please arrive 15 minutes earlier than your arrival time so that you have time to complete the forms in the office. Please bring your photo ID, Covid-19 vaccination card, insurance card, and method of payment for any applicable copayment, coinsurance, or deductible (due on the day of service).
- Please note that masks are required in the office. Children cannot be left unattended in our office, and Dr. Schulman’s staff cannot monitor minors during the testing process. Please be prepared to turn off your cell phone or put it into airplane mode to avoid any disruptions during the test. We ask that all patients refrain from bringing into the office any knives, including pocket knives or utility knives.
